Changing your name or address – If you need to change your name or address, please provide proof of the change to protect your confidentiality and ensure your record is correct.
You can:
- bring proof to reception, or
- send a copy to the practice (do not post original documents).
Examples of acceptable documents include deed poll/marriage certificate (name change) or a recent utility bill/bank statement/council tax bill/tenancy agreement (address change).
